Accident Number: WPR16FA111
Date & Time: 05/15/2016, 0829 PDT
Registration: N133BW
Aircraft: CESSNA R182
Aircraft Damage: Destroyed
Defining Event: Controlled flight into terr/obj (CFIT)
Injuries: 1 Fatal
Flight Conducted Under: Part 91: General Aviation - Personal
The pilot, an instrument-rated private pilot, departed on a cross-country flight in daytime IMC conditions. Initially in contact with ATC, the pilot received heading and altitude instructions as the flight neared its destination. Despite compliance early on, communication issues arose when the pilot did not respond to subsequent instructions to turn and climb. Attempts to reach the pilot were unsuccessful before radar contact was lost. The airplane ultimately crashed into mountainous terrain, consistent with controlled flight into terrain. The wreckage was severely damaged, hindering examination of onboard avionics that could have aided situational awareness. While the pilot had severe coronary artery disease and tested positive for hydrocodone, it couldn't be determined if these factors contributed directly to the accident. The NTSB concluded that the pilot's failure to maintain terrain clearance in IMC conditions, compounded by communication failure, led to the accident.

The guy probably had a heart attack. His speech and tone were normal on his last transmission. If it was carbon monoxide, he would have sound drunk and nonsensical. He probably had autopilot which kept him on the steady coarse til the end.

I gotta say to me it looks like pilot incapacitation

I passed out while driving. Went off the road and fortunately into a farm field. Came back to still moving at 45 mph. Turned out to be blood pressure meds driving my BP too low. The physician misdiagnosed " white coat syndrome" as being low blood pressure. It took 3 months ot passing out every time I coughed, which the med also was causing, and 15 physicians to finally figure out the problem. These types of circumstances are why so many probable causes are inaccurate.

So sad. I feel for the controller and family of the pilot 🙏🏾

Never got SoCal approach freq dialed in right or he had a headset malfunction.
-
Got so distracted by comms issues he stopped navigating imo

Shouldn’t there be an attempt with 121.5?
-
was this a drug plane?
-
I bet anything it’s pilot incapacitated….his voice sounded to calm and very deliberate on his last few transmissions…carbon monoxide, maybe the guy suffered a stroke or heart attack….maybe suicide too… maybe never know
-
Could be radio reception for the area that he was in!
