A cinch on a horses saddle is the main strap that holds the saddle onto the horse 🐴 and the noise the horse makes is their little complaint against the tightening of that strap 🐴

Also , since gray wolves are adapted to chase prey over long distances, they have lots of adaptations to increase their stamina in order to hunt for many hours, so Ghost is probably doing okay with chasing Jon.
